Our goal is to empower people to navigate the complex healthcare system and improve their health outcomes.

OUR VISION: The Healthcare Navigation Project added to the NCQA* PCMH*/ and specialty home models and accredited as independent public healthcare education, health equity, youth health care transition, and given credit as the sixth point to the current five point STAR* system.

*STAR system (Sustainable Tracking Assessment and Rating System) determines the degree of rebates an organization receives by following The National Committee For Quality Assurance (NCQA) and The Patient Centered Home Model (PCMH).
