Patient navigation is a CDC policy strategy that employs a Patient Navigator for whole-patient care through intensive case management. Patient Navigators are sometimes referred to as Parent Navigators but are employed by a hospital system, physician office and others, and are bound by HIPAA and the individual policies of their specific employer and state. They follow the Patient Centered Medical Home Model. They work one-on-one with clients to encourage continued commitment and adherence to medical treatment, access to social services, improved communication, and prompt re-engagement in care. They call to remind you of your doctors appointment and will come and pick you up at home if you need transportation. They are funded partially through rebates given to these entities for adhering to government policy.

We are PARENT navigators…..lay people who are independent subject matter experts with lived experiences navigating systems on behalf of a child, youth or young adult with health challenges. These include physical, behavioral, and mental health challenges, as well as complex care needs. They have experience working with families, are familiar with the resources in your community, region and the federal level. They understand relevant systems, services and processes. (e.g., Individualized Education Plan (IEP) and 504 developments. They have strong interpersonal skills, empathy and work with communities of diverse socioeconomic and cultural backgrounds.

We are an independent, immersive and interactive physical and mental healthcare literacy and life skills program, designed for ALL youth transitioning to adult care, families, caregivers, employees and everyone else. We work with those with chronic disease, underserved, overserved, those with educational disabilities. We fill the gap for the 9 out of 10 who have limited health literacy.
