Lauren Kopsick and Ivy Braun are the founders of the Healthcare Navigation Project (THNP), formerly known as Parent It Forward prior to winning The CODY Award for Nonprofit of The Year by The Charity Design Company with formal launch Thanksgiving 2022. This is the first independent public healthcare literacy and life care program for all youth as they transition to adult care and their families. They share their advice for transition planning after pediatric care.
